∙수출자의 이름, 주소(국가포함), 사업자등록번호(칠레: Unique Tax Number) 기재.

∙생산자 1명일 경우: 생산자 이름, 주소(국가, 전화번호, FAX, E-mail 포함), 사업자등록번호 기재

∙생산자 2명 이상일 경우: “VARIOUS”를 기재하고 모든 생산자의 리스트를 첨부(생산자 이름, 주소, 국가, 전화번호, FAX, E-mail, 사업자등록번호)

∙생산자와 수출자와 같을 경우: “SAME” 기재

∙생산자를 모를 경우: “UNKNOWN” 기재

∙비밀로 할 경우: “Available to Customs upon request” 기재

∙수입자 이름, 주소(국가포함) 기재

∙수입자를 알지 못할 경우: “UNKNOWN” 기재

∙수입자가 다수일 경우: “VARIOUS” 기재

Preference Criterion

(특혜기준)

∙항목4의 각 물품에 대하여 적용할 수 있는 특혜기준(A~D) 기재

  A: 역내국에서만 완전하게 획득하거나 생산된 경우.

  B: 물품이 일방 또는 양 당사국의 영역내에서만 생산되고 해당 물품의 생산에 사용된 비원산지재료에 대해 부속서(annex 4)에 규정된 세번변경기준, 부가가치기준, 주요공정기준 및 협정 4장에 규정된 다른 적용가능한 기준을 충족한 경우

  C: 물품이 협정 제4.2조 제1(a)부터 제1(d)의 기준에 의한 원산지요건을 충족한 원산지재료로만으로 일방 또는 양 당사국의 영역내에서만 생산된 경우

  D: 물품이 일방 또는 양 당사국 영역내에서 생산되었으나 그 물품의 생산에 사용된 하나 이상의 비원산지재료가 협정 4.2.1 C
(i, ii) 규정에 해당하여 세번변경은 이루어지지 않았으나, 부가가치 기준을 충족한 경우
